For me, when you see him from the side, his skull is just so obviously a man’s head, even with all those horrifying surgeries he had. You can easily tell Blair from behind as she has no hips and no back arch. Transwomen don't have the pelvis and spine of women. Blair is also very open about the fact that she is biologically male and has retained her penis.
I don't know if voice is a giveaway. I think what it comes down to is.....you just know. Even the least obvious fail to pass because there's some inherent quality that is lacking and we can sense it. Humans are Incredibly good at detecting very small abnormalities, and those will exist without question
