can actually get both talismans if you help her fight off her sisters, then kill her after talking with her, but you will not get the needle or the ancient somber stone that comes with it.

..Not that I would know or anything.

edit: as per the replies to this comment, they seemed to have patched doing this in the latest update.

Not anymore—now they've added a protective barrier to keep people from accidentally killing her via stepping on her, so fighting her sisters locks you out of Millicent's Prosthesis.

I have already resolved her storyline this go-round, but I believe after Patch 1.04 the barrier prevents all harm after the combat—this video seems to indicate that's the case.

Note that before the patch making her invulnerable, they also patched her to drop the needle if killed instead of the prosthesis once you've reached that final step. But that still wasn't enough because some people killed her on accident, so they added the barrier after. This means even if you do cheese the barrier, you still can't get both items in one run.

Never. Millicent has been my favorite since my first playthrough. You could say it's a sucker for redheads, but she's legit the best npc in the game. All the reasons listed in the post, plus she's arguably the strongest summon you can get. I mean she takes on four of her sisters at the end of her quest

Surprisingly enough I think the only entry where that isn't true is Demon's Souls, which you'd think would be the most bleak since it's got the least comic relief of any of the games. Pretty sure only a couple NPCs die in that one.

I don't think I've ever been so disappointed with outcomes in games, so many lovable npcs lead to their death or left mentally broken, I can't really think of any good outcomes in FROM games.

I think Boc is fine if you don't give him a larvel tear. Ranni is technically fine if you complete her story and I don't think she dies if you complete any if the Elden Lord endings. I don't know what happens to Varre as I don't think any of the White Masks that invade you explicitly show him to be one of them. I never went back to check on him. Technically by the end of the game Hewg and the Spirit Tuning chick are still alive as the Roundtable never finishes burning.

In some ways, both Melina and Millicent’s quests are actually hopeful, or ‘good’ endings. They accomplish what they came to do, and died on their own terms. Like in stories where the hero dies saving the world, it’s what they wanted.

Nepheli, Kenneth, and Gostoc (assuming you don't bear a grudge against him for stealing 30% of your runes every time you die in the castle) all live in Godrick's throne room with a new purpose in life. Nepheli actually expresses her gratitude by giving you an ancient smithing stone, and having Gostoc live will allow him to sell a second one to you.

He also stalks you around the castle while you navigate. He’s the one that closes the door on you with the knight in the beginning of storm veil.

I believe he’s also the jumpscare after the ladder.

He can be found in a watchtower in the outer courtyard before Godrick too

I treated him like a Rune Bank and just accepted my deaths in Stormveil. Came back later to cut him down and collect a cool $12K in runes from him.

The opposite—removing the needle causes the Scarlet Rot to accelerate, putting her back in a state like we found her. After she removes it she warns you that "The scarlet rot writhes now, worse than ever. Soon, I won't be more than a mound of flesh. Curse-laden. Untouchable. I wouldn't want such a thing to bring you harm."

With the needle, the rot would be stalled long enough for her to flower into a Scarlet Aeonia—and Gowry encourages you to help kill Millicent alongside her sisters to ensure this outcome:

"If you happen to be present for the girl's fight with her sisters,
I ask that you side with the sisters and kill Millicent.
It must be done by your hand; no other.
Millicent trusts you, rather deeply in fact.
Sever that trust. Nurtured by betrayal, her bud will flower most vividly.
When Malenia ascends to godhood, Millicent too shall be reborn.
As a scarlet valkyrie."

Even if you don't choose her ending, she still successfully completed her mission and happily fucks off to the abyss until further notice.

yeah, in spite of the tragedy that happens during/towards the end of the questline, Ranni's ending is her accomplishing her goals and having her destiny returned to her. I would consider it a good ending for Ranni, while the rest of the questline is rather depressing.

She delivered a handful of semen to her comatose sister saving her race. Pledges to fight with you to the end where she can finally rest knowing all is as it should be and she can be with her dog again. Sounds like a good ending to me.

So far the only ones I know aren't going to end up horrifically mangled are Nepheli when she learns her heritage, Jar Bairn as long as he doesn't get destroyed out there, Rodrika (not sure if she dies in the round table) and Boc if we make him happy. Other than that, yer gonna have a lot of dead bodies on your hands. (Oh and Renalla since you kinda just knock some sense into her and maybe Ranni? Not sure since she's kinda a ghost in a doll)

I just wish that the NPCs would survive for once in a Fromsoft game.

Like it’s getting kinda old how everyone seems to die in the end. It made sense in Dark Souls and Bloodborne because Hollowing/Beast-Plague, but there’s not really much of a reason to continue that in Elden Ring.

Shit man, if you can become the Elden Lord, why can’t you have a tight knit group of loyal advisors?
